Hey bud,

You use the Kin address. So your home address.

April Address is like the house you live in summer.. so don't worry about that unless you do live in another house in the summer.

Secondary School one is the address of your school - so like if you're in a boarding school.

The "Online Help" section helps a lot, and I believe ur supposed to fill in all categories except "secondary school address" (unless ofc ur away to attend school). So ya lmao* sums it up very well for u.

It comes up blank for me too.

Rather than opening the pdf in your browser, save it to your desktop and then open it. Worked for me that way.

It comes up blank for me too.

As well you can try using Internet Explorer, that fixed the blank problem for me!
