For certain scholarships you need put references information like address, phone number, and email. If you use a teacher as a reference, can you use the school's address and phone number or do you have to ask for your teacher's personal home address and phone number?

Ask for teacher's school extension number(5 digits for my school)
I just put the school's address and number because if they really do call the school, the secretaries at the office can easily extend you over to the teachers' classrooms.
